Provides assessment, diagnosis, and treatment of communication, cognitive, and swallowing disorders in patients. Develops personalized treatment plans and collaborates with healthcare teams to enhance patient outcomes. Responsibilities:
- Evaluate patients' communication, cognitive, and swallowing functions using standardized assessments.
- Develop and implement evidence-based treatment plans tailored to individual patient needs.
- Monitor and document patient progress, adjusting treatments accordingly to ensure optimal outcomes.
- Educate patients, families, and caregivers on therapy techniques and strategies.
- Collaborate with multidisciplinary teams to coordinate care and communicate patient progress.
- Complete comprehensive discharge summaries and maintain accurate electronic health records in adherence to regulatory standards.
- Hold active ASHA certification and applicable state licensure.
- Possess a minimum of 1 year of speech-language pathology experience, preferably in LTC or rehab settings.
- Current Texas State License required.
- Certification in BLS.
- Demonstrate strong clinical assessment and documentation skills.
- Ability to supervise and support CFY or student SLPs is preferred.
