Our fully integrated care model lets you focus on delivering high-quality care while working alongside a collaborative, multidisciplinary team on-site and across the system. PCHC’s services include behavioral health, dental care, primary care pharmacy, rehabilitation, substance use disorder treatment, case management, chronic disease support, and more—all designed to improve outcomes and strengthen the communities we serve. Physicians here serve not only as caregivers, but also as leaders and advocates who advance population health and support underserved communities. Advanced Practice Provider Highlights
  • Flexible full-time or part-time schedules: 4–5 days per week, Monday–Friday
  • No routine evenings or weekends
  • Shared telephone call approximately one week every six months, with nurse triage support
  • For full-time schedules: 34 patient-care hours and 6 administrative hours per week
