Together, they deliver personalized, high-quality care.Why You'll Love Working HereTeam-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness.More Time with Patients: See fewer patients per day and spend more time delivering meaningful care.Supportive Culture: We foster a welcoming, inclusive environment where teamwork and growth are prioritized.Your Role & ResponsibilitiesYou will provide comprehensive care to seniors following clinical standards.You will collaborate daily with your care team to ensure coordinated, high-quality care.You will manage referrals, hospital/SNF coordination, DME, and home health services.You will document care with support from quality-based coders.You will participate in on-call rotation and support growth plans at your center.You will have experience providing value-based care.Use Your Skills to Make an ImpactWhat We're Looking ForYou will have a Master's Degree in Nursing or completion of a PA program with board certification.You will have an active, unrestricted NP or PA license in the state of practice.If selected for this role, we will require you to undergo TB screening.Spanish speakingPreferred:You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and Medicaid and Medicare numbersAdditional Information:Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. The Federal Government will require applicants to take the Interagency Language Rating (ILR) test.You'll work onsite at the State Ave. clinicYou'll report to the Area Medical DirectorScheduled Weekly Hours: 40Pay Range: $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan.
