Licensed Practical Nurse (LPN) – Permanent

Berlin, NH
Full-Time | 12-Hour Nights

Rate: $31.35–$36.75/hr + Excellent Benefits

Position Details

• Position: Licensed Practical Nurse (LPN)
• Employment Type: Permanent
• Location: Berlin, NH
• Shift: Nights (7:00 PM – 7:00 AM)
• Schedule: Three 12-hour shifts per week
• Every other weekend rotation
• Union position

Requirements

• Graduate of an accredited Practical Nursing program
• Current New Hampshire LPN License
• Current AHA BLS Certification
• Long-term care or skilled nursing experience preferred

Key Responsibilities

• Provide skilled nursing care in accordance with physician orders, resident care plans, and established standards of care.
• Assist with assessing, planning, implementing, and evaluating resident care needs.
• Administer medications, treatments, and nursing interventions as ordered.
• Monitor resident conditions and communicate changes appropriately to physicians and nursing leadership.
• Supervise and support LNAs/LMNAs during assigned shifts.
• Assist with resident admissions, transfers, discharges, and care coordination as needed.
• Collaborate with physicians, residents, families, and the interdisciplinary care team.
• Complete accurate and timely clinical documentation.
• Ensure compliance with state regulations, facility policies, and professional nursing standards.
