Registered Nurse or Medical Social Worker - Case Management Assignment Duration This is a 13-week contract position. Schedule Day shift, 5 days a week from 07:30 AM to 04:00 PM, with weekends required every other weekend. Job Description We are seeking a skilled Registered Nurse or Medical Social Worker with acute care experience to join our Case Management team. The primary focus will be on discharge planning and care coordination within a Level II Trauma Center. The candidate must have experience working across various units, including the emergency department and medical-surgical settings. Responsibilities
- Coordinate discharge planning and ensure smooth transitions for patients.
- Conduct thorough assessments and evaluations related to patient care needs.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Assist patients in accessing community resources and support services post-discharge.
- Maintain accurate and timely documentation in the EPIC charting system.
- Minimum of 1 year of acute care experience in a hospital setting.
- Experience in discharge planning and care coordination.
- Current RN Licensure or MSW in the state, with BLS certification required.
- Strong preference for candidates with diverse case management experience.
