Join our team and elevate your career by supporting skilled nursing, long term and memory care or rehabilitation services.As an RN in our beautiful Long-Term center on the Chesapeake Bay, you will play a pivotal role in coordinating clinical care, and ensuring exceptional resident outcomes. This is not just a job—it's an opportunity to foster innovation, and shape the future of healthcare delivery.What You'll Do In This RoleRecognize and inform the appropriate physician of significant changes in the resident's health status and for adverse reaction(s) to medication.Maintain medication and resident's records.Participate in resident assessment and care planning reviews and revise residents' assessments and care plans.Insure problems and complaints involving nursing services are reported to the Team Leader in a timely and appropriate manner.Communicate and coordinate care with residents and their families/significant others.Document and lead others in the documentation of resident care.Direct, participates and/or assists staff in delivering care.Check for appropriate amount and condition of equipment and supplies; order supplies and equipment as necessary.Support and participate in facility quality assurance programs.Ensure that unit practices are consistent with regulatory requirements.Review, transcribe and implement physician orders.Practice and ensure staff adheres to infection control measures.Participate in continuing education programs to maintain competency.All other duties as assigned.Core CompetenciesSupportive and Compassionate (to both patients and nursing staff).Regularly exercises sound and professional judgment.Excellent verbal and written communication skills.Physical RequirementsAbility to climb stairs, stand or walk for extended periods.Must be able to lift up to 25lbs.The Experience You'll BringRequiredActive, unencumbered RN license in the State of Virginia (or compact).A minimum 2 years of nursing experience.Must be proficient with all software applications and systems utilized in The Hoy Center.
