MDS Coordinator RN

$5,000 Sign On Bonus

Salary: $115,000 to $125,000 DOE

Join our team as an MDS Coordinator RN and make a meaningful impact on resident care. We are seeking an experienced Registered Nurse to oversee the MDS process, ensure regulatory compliance, and collaborate with our interdisciplinary team to deliver exceptional care.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time

• Develop, update, and maintain resident care plans

• Manage Medicare and Medicaid assessment schedules and documentation

• Ensure compliance with all federal, state, and facility regulations

• Collaborate with physicians, nursing staff, therapy, and other interdisciplinary team members

• Maintain accurate resident documentation and medical records

• Participate in quality assurance initiatives and regulatory surveys

Qualifications

• Current, active RN license in the state of practice

• MDS experience preferred

• Knowledge of Medicare, Medicaid, and skilled nursing regulations

• Experience with PointClickCare (PCC) preferred

• Strong organizational, leadership, and communication skills

• Current CPR certification preferred
