Job Title Behavioral Health Specialist – Requires LCSW, LPC or LMFT Company Oak Street Health Overview Our Behavioral Health Specialist provides care management services and evidence‑based short‑term psychotherapy to patients with behavioral health needs. The role coordinates care across primary clinics and works with care teams and psychiatric consultants. Core Responsibilities Collaborate with primary care teams to identify and refer patients to the Oak Street Behavioral Health Program. Screen and assess patients for common mental health and substance use disorders. Develop care plans and facilitate referrals to Oak Street’s psychiatric consultant or external resources. Provide short‑term counseling and evidence‑based treatments. Coordinate care with psychiatric consultants, including warm handoffs for telehealth visits, and collaborate on care plans. Develop relationships with primary care teams and consistently communicate changes in behavioral health care plans. Accurately document patient encounters and counseling sessions in the electronic medical record. Participate in regional and organizational efforts to advance the Behavioral Health Program through workshops, feedback sessions, and surveys. Cover multiple clinics as needed. Qualifications Master’s Degree in mental health counseling/healing arts from an accredited school (Social Work, Counseling, Marriage & Family Therapy, etc.). Highest clinical level of license (or local equivalent) in the U.S. Experience screening for common mental health and/or substance use disorders. Experience assessment and treatment planning for these disorders. Familiarity with brief, structured intervention techniques (Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.). US work authorization. Proficiency in brief, structured intervention techniques. Working knowledge of differential diagnoses of common mental health and/or substance use disorders. Proficient PC skills. Pay & Benefits Anticipated weekly hours: 40 Pay range: $66,575.00 – $142,576.00 (base salary). Eligible for bonus, commission, or short‑term incentive program.
