Job Summary

We are seeking three dedicated and experienced Case Managers to join our acute care team at Morristown, NJ. This role is critical in ensuring seamless patient transitions from inpatient hospitalization to appropriate post-acute care settings. The ideal candidate will have strong clinical judgment, excellent communication skills, and a proven track record in discharge planning, utilization management, and quality screening within an acute care environment.

Key Responsibilities

·       Coordinate comprehensive care for patients during acute hospitalization

·       Lead all aspects of discharge planning and patient transition to rehabilitation, skilled nursing, or home care settings

·       Perform utilization management and quality screenings to ensure appropriate use of healthcare resources

·       Collaborate with multidisciplinary teams, including physicians, social workers, therapists, and payers

·       Maintain accurate documentation in compliance with regulatory and accreditation standards

·       Manage care for subacute and complex patient populations

·       Rotate weekends every 4th or 5th weekend; cover one summer and one winter holiday

Required Qualifications

·       Bachelor of Science in Nursing (BSN)

·       Active, unrestricted Registered Nurse (RN) license in the State of New Jersey

·       Minimum of 2 years of acute care experience with demonstrated discharge planning and placement expertise

·       Proven experience in inpatient case management within a hospital setting

·       Experience working with rehabilitation facilities, skilled nursing, and community-based services

·       Strong organizational, interpersonal, and problem-solving skills

·       Proficiency in electronic health records and care management software

Preferred Qualifications

·       Certification in Case Management (CCMC)

·       Background in managed care environments

·       Experience with complex medical and surgical populations
