We are seeking a compassionate and experienced Registered Nurse to join a growing Home   Health team serving the Dickson City, PA area. This full time opportunity is ideal for nurses who enjoy working independently, building strong patient relationships, and delivering high-quality care in the comfort of patients' homes.

Responsibilities:
• Perform patient assessments and develop individualized care plans
• Provide skilled nursing care in patients' homes
• Educate patients and families on disease management and treatment plans
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Complete timely and accurate documentation
• Help patients remain safe, healthy, and independent in their homes

Experience and Skills Needed:
• Active RN license
• Home Health experience required
• OASIS experience strongly preferred
• Strong clinical assessment and communication skills
• Ability to work independently and manage a field-based schedule

Pay Details: $87,000.00 to $103,000.00 per year
