Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to meet the diverse mental health and medical needs of patients in the primary care setting.

• Provide a range of behavioral health services including individual and group counseling, crisis intervention, intake interviews, diagnosing, and treatment planning.
\n
• Document all assessments and consultations electronically, ensuring meticulous record-keeping in compliance with state regulations and company protocols.
\n
• Facilitate collaboration with healthcare providers by escalating patient needs and connecting clients to appropriate services, including medication assistance and community resources.
\n
• Participate in the Quality Improvement teams to develop and implement processes for data collection and outcome evaluation.

Patient Population / Client Focus\n

In this role, you will engage with a diverse patient population that includes individuals facing mental health challenges, substance use disorders, and a variety of psychosocial stressors. Your expertise in offering culturally sensitive and age-appropriate care will be essential in addressing the specific needs of each client you work with.

Qualifications\n

• Must hold a current and valid state license as a Licensed Clinical Social Worker (LCSW).

• A Master's degree in Social Work (MSW) is required along with a minimum of three (3) years of post-licensure clinical experience.

• Demonstrated knowledge of effective counseling techniques, crisis intervention strategies, and the ability to make independent judgment as part of direct service.

• Strong skills in group therapy facilitation and the ability to effectively plan therapeutic processes.
