The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy. What You’ll Do Conduct Occupational Therapy Evaluations to establish individualized plans of care Evaluate patients’ functional abilities related to activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Develop and implement personalized treatment plans to improve independence, safety, and quality of life Provide therapeutic interventions addressing self-care, home management, cognition, and upper extremity function Educate patients and caregivers on adaptive techniques, energy conservation, home safety, and use of adaptive equipment Collaborate closely with physicians, nurses, physical therapists, speech therapists, and other care-team members to coordinate care Monitor patient progress, update care plans, and maintain thorough, compliant documentation Ensure compliance with all Medicare, CMS, and home health standards What We’re Looking For Current Florida Occupational Therapist (OT) license (required) Previous home health experience is strongly preferred but not required Excellent communication, time management, and documentation skills A compassionate, dependable, and professional attitude A team-oriented mindset and a genuine passion for making a difference in patients’ lives What Sets Us Apart A true culture of teamwork and family: we support one another and celebrate each success together A patient-first approach that respects patient preferences and promotes dignity Work-life balance with flexible scheduling, so you can care for your patients and your life outside of work A reputation as the “best in home healthcare” in our area, built on trust, quality of care, and long-standing relationships with patients and families Are you ready to join an Awesome Team?
