Physical Therapist

Overview Klarus Home Health is seeking a PRN / Per Diem Licensed Physical Therapist to provide PT services to patients enrolled in our home health program in the Dallas/Dallas South region and surrounding areas. Responsibilities Evaluate patients\' physical and functional abilities to develop individualized care plans. Plan, implement, and modify physical therapy treatment programs based on medical referrals and assessments. Provide hands-on therapy, exercises, and techniques to improve mobility, relieve pain, and restore function. Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention. Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans. Participate in ongoing professional development to stay current with trends and best practices in physical therapy. Willingness to perform a Saturday rotation approximately once every 4–6 weeks. Qualifications Graduate of an accredited School of Physical Therapy. Current State license as a Physical Therapist, or a multi-state PT Compact license if practicing in a PT Compact state. Minimum of two years of experience as a Physical Therapist in a clinical care setting; Home Health, geriatrics, or medical-surgical experience preferred. Excellent interpersonal and communication skills. Strong nursing skills as defined by generally accepted standards of practice. Reliable transportation with valid auto liability insurance. Ability to work flexible hours and travel locally as needed. Ability to lift up to 50 lbs. and perform physical tasks related to patient care.
