Overview Speech Therapist – Mobile Outpatient Therapy (Full‑time) in Westfield, MA. The Speech Language Pathologist will provide in‑home outpatient rehabilitation services for older adults, evaluating and treating communication and swallowing disorders in a patient’s private residence or assisted‑living facility. Responsibilities The Speech Language Pathologist organises and delivers speech‑language services, directing patient participation to restore communication and swallowing functions. Responsibilities include: Supervise CFY temporary licensees, students, and support personnel in accordance with state licensure and professional standards. Adhere to department and patient treatment schedules, complete timely clinical and billing documentation, and attend staff meetings. Contribute to patient care and staffing conferences and related meetings. Make referrals to other disciplines and agencies as needed. Consult with the rehabilitation team and physician to select appropriate therapeutic regimens. Adapt program and care plans to individual needs. Promote a safe environment and enforce fire and safety hazard elimination. Order supplies and equipment as necessary, and maintain equipment condition. Prioritise patient service, ensuring residents and families receive high‑quality care. Screen, examine, and evaluate patients, including history and systems review; apply appropriate tests and measures, interpret findings, and establish diagnoses. Develop treatment goals and methods in collaboration with patients and caregivers, implement treatment plans, and complete all related documentation. Coordinate acquisition of adaptive communication devices and instruct patients in their use. Facilitate identification of hearing deficits and refer as appropriate. Select appropriate activities suited to the individual’s capacity and interests. Instruct patients in skills and techniques for therapeutic activities and evaluate progress. Perform routine re‑examination to modify or progress treatment plans. Coordinate care in home and community with team and physician in collaboration with referring physician. Perform other related duties as required.
