Medicare-certified home health agency seeking a Physical Therapist for a full-time position serving older adults in the Martin and St. Lucie areas. The role offers a competitive salary, health insurance at little cost to the employee, life insurance, paid time off, and mileage reimbursement. Candidates must have a clean driving record, reliable transportation, and successfully complete a Level II background screening through Florida Clearinghouse
Responsibilities
- Evaluate older adult patients in the home health setting.
- Develop individualized treatment plans.
- Monitor patient progress and adjust care as needed.
- Coordinate care with patients, families, and other healthcare professionals.
- Complete accurate clinical documentation.
Required Qualifications
- Clean driving record.
- Reliable transportation.
- Successful completion of a Level II background screening.
Preferred Qualifications
- At least one year of home health physical therapy experience.
- Experience conducting physical therapy evaluations.
- Experience using electronic health records.
- Knowledge of Medicare documentation requirements.
- Experience coordinating patient care.
