Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 2: Atlantic Beach, Baldwin, Cedarhurst, East Rockaway, Bellmore, Freeport, Hewlett, Inwood, Island Park, Lawrence, Levittown, Lido Beach, Long Beach, Lynbrook, Merrick, Oceanside, Point Lookout, Rockville Centre, Seaford, Valley Stream, Wantagh, & Woodmere.

• 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Our Home Care LPN's:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.

Minimum Qualifications:

• New York State Licensed Practical Nurse (LPN) license
• Minimum of one year of recent hands on experience
• Basic Life Support Certification

Salary Range USD $33.00 - USD $40.00 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
