Monitor and report on key metrics and care outcomes. Assess patient needs and determine appropriate risk-stratified case management. Collaborate with interdisciplinary teams to optimize care pathways. Develop and update individualized care plans aligned with evidence-based guidelines. Ensure compliance with regulatory requirements and documentation standards. Utilize caseload pipelines to forecast workload and target outreach effectively. Advocate for patient needs and coordinate community resources. Educate patients and families about treatment options, expectations, and planning. Identify and pursue opportunities for process improvements and efficiency gains. Maintain up-to-date knowledge of care management best practices and guidelines. Experience/Qualifications Registered Nurse (RN) license with current Georgia/State-specific licensure. Clinical experience in case management or care coordination, 2+ years. Ability to assess risk, stratify patients, and develop individualized care plans. Experience with documentation standards and regulatory compliance. Proficient in evidence-based guidelines and care pathways (e.g., CMS, JCAHO). Strong communication and collaboration with interdisciplinary teams. Competence with caseload pipelines, analytics, and workload forecasting. Bachelor’s degree in Nursing or health-related field preferred. Patient advocacy skills and ability to coordinate community resources. Demonstrated process improvement mindset with track record of efficiency gains. What's in it for you?
