Please review the below job description and Apply Now if you are interested. **What will Flexible Scheduling Offer Me?** - Extended Hours Monday-Friday starting 7:30a-7p - Extended Hours Saturday & Sunday starting 7:30a-7p - Nurse Assessor Can Build Their Own Schedule Each Week - Nurse Assessor Can Make Up Hours for Interrupted/No Shows - Work Any Calendar Day **Pay Rate: $58-$60/hr.** _Hourly wage may depend upon experience, education, geographic location and other factors._ **Qualifications** - New York State RN License, required. - Work experience with UAS-NY or Oasis Community Assessment tool - 2+ years' experience working in-home setting within the last 15 years, required. - Work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities. - Computer literacy: ability to navigate new and emerging technology; ability to perform telehealth assessment; highly developed written and oral communication skills; strong interpersonal skills; and proven ability to work with community groups. - Knowledge of long-term care services and programs - Ability to travel throughout the assigned region, as assigned, including travel to individuals' homes. **Highlights** - Full county coverage required. - Appointments based on proximity to your home/self-assign your territory. **Responsibilities** - The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, to assist the Physician in the completion of a Physicians order. - The incumbent may follow up with individuals who have received an assessment through face-to-face communications. - Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports. - Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes. - Work collaboratively to monitor and address clinical quality and gaps in care. - Provide clinical and educational assistance to support project initiatives. - Demonstrate communication skills necessary to interpret age-specific responses.
