Case Management Nurse

Job Overview We are seeking an experienced Inpatient RN Care Manager to support hospitalized patients through safe, efficient, and timely transitions of care. This role focuses on coordinating clinical, psychosocial, and financial aspects of care while collaborating closely with interdisciplinary teams to optimize patient outcomes and resource utilization.
