Hospice RN Case ManagerAthena Home Health & Hospice, a leading provider of continuum of care services in Southern New England, is seeking a compassionate and skilled Hospice RN Case Manager. As a vital member of our hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.This position is on-the-road covering Litchfield County and western Hartford County. Salaried role with weekly pay up to $49 an hour, depending on experience. Shift is Monday-Friday 8a-4:30p. Position is vacant due to relocation.Responsibilities:Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing processComplete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluationsDevelop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actionsCounsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriateAct as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseloadRequirements:Graduate of an accredited school of nursingOne (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home careActive and unencumbered RN license in ConnecticutCPR certificationBachelor's degree, with one (1) year of hospice care experience preferredBenefits:Competitive and Weekly PayHoliday PayCareer Advancement OpportunitiesExclusive Employer Discount ProgramBenefits Available for Eligible Team Members:Employer Paid Life Insurance401(k) with Employer MatchVacation and Personal TimeHealth, Dental, and Vision InsuranceWe are an equal opportunity employer that values diversity at all levels.
