Position Overview

This position involves providing comprehensive home health care to patients in their residences. The contractor will be responsible for patient visits, documentation, and coordinating care as part of a healthcare team.

Contract Assignment Details

• Duration: 13 weeks
• Schedule: Monday to Friday, 8 AM to 5 PM
• Weekend Rotation: None
• On-Call Responsibilities: None
• Region: Home Health
• Territory Covered: Greater Manchester area
• Weekly Travel: Approximately 300 miles
• Productivity Expectations: 30 units per week

Requirements

• Experience: Minimum of one year of recent adult home health or hospice experience required.
• Certifications: Must hold valid CPR and BLS certifications.
• Travel: Must be willing to travel up to 75 miles from the assigned clinic.
• Transportation: Reliable transportation and a current driver's license are essential. Liability insurance must be maintained.

Responsibilities

• Conduct home visits and provide patient care as per individualized plans.
• Document all patient encounters accurately and promptly within 24 hours.
• Attend periodic office meetings to discuss patient care and team collaboration.
• Accept all patients assigned to the schedule, regardless of distance.
• Coordinate services and manage patient care activities, including assessments and care management.

Using Technology

All clinicians must be proficient in the Homecare Homebase (HCHB) EMR system. Experience with this system within the past six months is required for applicants.

Work Environment

The position involves working in various patient homes. There are no guaranteed hours, and contractors may need to float to nearby clinics if patient census is low. The expected daily caseload averages 5-10 patients.
