FreedomCare in New York City is seeking a Health Home Care Manager to join our Care Management team and coordinate patient-centered care at home. You will screen eligibility, develop care plans with providers, and coordinate services across the health system while maintaining patient confidentiality. Travel across Brooklyn, Bronx, Manhattan and Queens is required to service chronically ill patients.
