Clinical Pharmacist

Overview Clinical Pharmacist (PRN) – Day shift, 12‑hour shift. Join the Tidelands Health team to help people live better lives through better health. The pharmacist works under general supervision to oversee the pharmacy’s supervisory, distributive, and clinical functions, ensuring accurate and timely review, processing, checking, and monitoring of physician orders, patient medication profiles, and completed prescriptions. The role includes routine daily rounds on assigned nursing units, providing drug information, recommendations, and interventions to physicians and other health‑care professionals. Responsibilities Provide drug education and counsel patients on appropriate drug use and drug discontinuation when indicated. Write patient drug education materials. Educate pharmacy staff, students, and other healthcare professionals. Utilize pharmacokinetic and drug dosing protocols and provide pharmacokinetic consultation for agents requiring monitoring. Evaluate drug therapy orders for appropriateness of dosage, route, interval, schedule, and duration throughout the patient’s hospital course and provide drug therapy consultations. Suggest appropriate therapeutic alternatives for non‑formulary drugs. Analyze patient parameters (weight, height, existing condition, and laboratory data) and recommend appropriate dosing. Participate in patient care rounds to provide input into monitoring of drug therapy. Report adverse drug reactions or medication errors. Record recommendations, interventions, or other appropriate activities in EPIC. Assist the Director and Assistant Director of Pharmacy in maximizing department performance and implementing and improving services. Conduct, perform, and present assigned projects to P&T and other committees as appropriate. Maintain competency by completing at least 15 Continuing Education (CE) credit hours per year, including 6 CE live credit hours. Qualifications Doctoral Degree from a College of Pharmacy. Previous experience in a hospital pharmacy. Registered Pharmacist – South Carolina Department of Labor, Licensed by the South Carolina Board of Pharmacy. Ability to work under direct or minimal supervision and handle stress appropriately. Collaborative teamwork with health‑system and pharmacy staff. Adaptability to frequent interruptions and changes in workload and schedule. Priority setting, decision‑making, and rapid response to emergency requests. Communication, supervisory, organizational, learning, and minimal teaching skills. Capacity to meet pharmaceutical care needs of neonatal, pediatric, adult, and geriatric patients. Physical Demand Light-Medium physical demand.
