Inpatient RN Care Management Fellow - Transition Expert

Providence Saint John's Health Center in Santa Monica is offering an RN Case Management Fellowship starting November 2, 2026. This full-time, day-shift program provides structured curriculum, experienced preceptors, and hands-on learning to help an RN transition into case management. The Inpatient RN Care Manager will assess and plan, coordinate care, manage resource utilization, and support discharge planning in an acute care setting, with the goal of delivering right care at the right time in #J-18808-Ljbffr Providence Health Plan Group
