Job Description Physical Therapist – Home Health Location: Port Charlotte / Punta Gorda, FL Job Type: Full-Time Compensation: $95,000 – $115,000 per year Benefits: Full Benefits Package Physical Therapist – Home Health Are you a Physical Therapist looking for more flexibility, independence, and earning potential without being stuck in a clinic all day? A growing Home Health Agency is seeking a Full-Time Physical Therapist to provide one‑on‑one care to patients throughout the Port Charlotte and Punta Gorda area . This position offers a competitive $95,000–$115,000 annual compensation package , a manageable territory, flexible scheduling, and full benefits. What You'll Do Provide skilled Physical Therapy services to patients in their homes Perform evaluations and develop individualized plans of care Help patients improve mobility, strength, balance, and functional independence Complete reassessments and discharge evaluations as appropriate Educate patients, families, and caregivers on exercises and safety Coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team Complete documentation accurately and timely manner Manage your daily patient schedule and territory efficiently What We're Looking For Current Florida Physical Therapist license Graduate of an accredited Physical Therapy program Strong clinical and communication skills Ability to work independently in a home-based environment Reliable transportation and valid driver's license Home Health experience is preferred, but qualified PTs from other settings are encouraged to apply OASIS experience is a plus Compensation & Benefits $95,000 – $115,000 per year Full-time position Comprehensive benefits package Paid time off Health insurance Flexible scheduling Local territory covering Port Charlotte / Punta Gorda One‑on‑one patient care Support from an experienced Home Health clinical team Why Home Health?
