RN Care Manager - Care Coordination & Discharge

Methodist Hospital

Methodist Hospital in Saint Louis Park is seeking a RN Care Manager to join our Care Management team. This role focuses on providing patient-centered care management services in the Emergency Center and inpatient settings, collaborating with healthcare teams and patients to ensure effective transition plans. The successful candidate will need a current Minnesota RN license and at least one year of hospital-based nursing experience, with a preference for experience in care coordination and discharge planning.
