Inpatient Registered Nurse - IPS RN

Purpose and Scope The Inpatient RN provides direct patient care in accordance with company policies and procedures, including FMCNA compliance programs and contracted hospitals’ policies and procedures. The role involves diverse areas of practice – hemodialysis, peritoneal dialysis, continuous renal replacement therapies, apheresis, and ultrafiltration. The Inpatient RN partners with the hospital to deliver quality care and a positive patient experience. Principal Duties and Responsibilities Under limited supervision, applies considerable knowledge to perform a broad range of tasks including direct patient care in multiple modalities such as hemodialysis, peritoneal dialysis, ultrafiltration, continuous renal replacement therapy, and apheresis. Provides supervision and direction of Patient Care Technicians and clerical staff; gives direction and education to ICU staff for CRRT. Directly cares for assigned patients in the Inpatient Renal Replacement and Apheresis Services setting, ensuring appropriate treatment techniques per modality procedures. Assesses patients’ responses to treatment, making adjustments to the treatment plan as directed by physicians, and communicates concerns to the Inpatient Program Manager, physicians, and hospital staff. Delivers safe, effective care in a timely and efficient manner. Provides organized, concise shift reports; applies advanced thinking to shift patient schedules to prioritize critical patients based on labs, MD orders, or assessments. Manages the implementation, administration, monitoring, and documentation of intradialytic transfusions, notifying physicians and blood suppliers of adverse reactions. Collaborates with physicians and healthcare team members to interpret, adjust, and coordinate daily patient care plans for continuity. Initiates and communicates with FMS facility nursing staff and hospital personnel as needed. Accurately documents all treatment information in the individual patient record (ACES) and the hospital record. Cleans and disinfects dialysis machine surfaces, chairs, equipment, and surrounding areas between treatments according to policies and procedures. Performs all tasks necessary for dialysis treatment preparation, including pre-treatment drum tests. Conducts manual or automated solution delivery system initiation when applicable. Operates related equipment safely, troubleshoots minor issues, and uses supplies efficiently to prevent waste. Conducts, documents, and verifies dialysis/apheresis machine safety tests, alarm tests, calibration, dialysate testing, functional testing, and disinfection of water and dialysis machines per FMS policies. Completes and documents ongoing participation in QAI activities. Participates in staff meetings and training sessions as scheduled or assigned. Serves as a subject‑matter expert for hospital staff on dialysis‑related patient/equipment complications. Reports all new or unusual incidents, information, complaints, or problems to the inpatient manager and other required parties per the Adverse Event Report requirements; may be assigned to in‑center patient care on an as‑needed basis. Reviews and complies with the Code of Business Conduct and all applicable company policies, local, state, and federal laws and regulations. Assists with various projects assigned by direct supervisor. Other duties as assigned. Physical Demands and Working Conditions The physical demands and work environment are representative of the essential functions, with reasonable accommodations for disabilities. Direct patient care involves interaction with patients, outpatient clinicians, hospital staff, and physicians, with travel between assigned hospitals and regional meetings. Position requires on‑call rotation including night, day, weekend, and holidays. Potential exposure to infectious material and contagious diseases. Position involves heavy lifting, moving patients or equipment over 200 lbs with assistance, and lifting chemical and water solutions up to 30 lbs 5 ft high. Supervision Provides supervision and direction of Patient Care Technicians (Certified Clinical Hemodialysis Technicians) and clerical staff; gives direction and education to ICU staff for CRRT. Education Current state licensure. Maintains current knowledge in nephrology, particularly inpatient care modalities. Meets practice requirements in the state of employment. Certification in Nephrology Nursing, Apheresis Nursing, or both is highly encouraged. Experience and Required Skills One year RN experience required with essential nursing skills. Minimum 6 months nephrology experience before independent dialysis services with no other Dialysis RNs. Minimum 6 months nephrology experience before independent apheresis services with no other Apheresis RNs. Demonstrated strong assessment skills for critically ill patients. ICU experience preferred but not required. Successful completion of training in ESRD patient care – Progressive Renal Education Preparation. Basic Life Support CPR recognition. Advanced Life Support CPR recognition preferred. Strong organizational and time‑management skills and commitment to ongoing renal/ nephrology education. Leadership skills and ability to supervise others such as PCTs, CCHTs, clerical staff. Successful completion of Nurses Technical Training.
