Community Director - LPN

Community Director – LPN Full-Time

Lead with compassion. Make a meaningful impact every day. At Fox Trail Memory Care, we provide personalized memory care in a warm, home-like setting where residents and families feel supported. We're seeking an experienced and compassionate Community Director – LPN to lead our team, oversee daily operations, and create a positive experience for residents, families, and staff.

Meaningful Impact: Make a difference in the lives of residents and their families every day

Competitive Pay: $75,000–$85,000 annually, based on experience

Schedule: Full-time, Monday–Friday schedule with flexibility to support community events and site needs as necessary

Supervise and support CMAs, CHHAs, and activities staff

Hire, onboard, train, and coach team members

Manage staff schedules to ensure adequate coverage

Complete resident assessments at admission, quarterly, and as needed

Oversee medication administration and clinical documentation

Ensure compliance with NJ DOH regulations and company policies

Conduct internal audits and quality checks

Manage resident records and staff files

Communicate resident condition changes to families and providers

Oversee meal supply ordering and home cleanliness

Lead monthly special events and support daily activities

Serve as the primary point of contact for families

Conduct tours and partner with marketing to maintain occupancy

Coordinate smooth move-ins and a welcoming environment

Active LPN license in the state of New Jersey

At least 1 year of experience in senior living, assisted living, memory care, or home care

Prior leadership, administrative, or management experience in a healthcare setting is required

Bilingual in Spanish preferred, but not required

Strong understanding of dementia and Alzheimer's care

Excellent organizational and communication skills

Proficient in Microsoft Office and willing to learn internal systems
