RN Home Health Travel Contract
Immediate need for a talented RN Home Health Travel Contract. This is a 12+ weeks contract opportunity with long-term potential and is located in Grand Rapids, MI (Onsite). Please review the job description below and contact me ASAP if you are interested. Job ID:26-28971 Pay Range: $65 - $70/hour. Subject to completion of scheduled hours.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Perform comprehensive patient assessments and develop/implement individualized care plans.
- Monitor patient conditions and communicate changes to the appropriate clinical team.
- Complete accurate and timely clinical documentation.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Educate patients and families regarding medications, treatment plans, and disease management.
- Follow home-health clinical, regulatory, and safety requirements.
- Complete required OASIS assessments and documentation.
- Maintain compliance with agency policies and applicable healthcare standards.
Key Requirements and Technology Experience:
- Active Registered Nurse (RN) license.
- Previous home-health nursing experience.
- Strong OASIS experience.
- SHA experience required/preferred based on client requirements.
- Excellent clinical assessment and documentation skills.
- Strong communication and patient-care skills.
- Ability to work independently in a home-health environment.
- Shift- Day shift - 4x10 or 5*8
