Our Hands of Hope Hospice team is looking for an Admissions RN! If you are looking for a rewarding career in hospice care and are passionate about providing high-quality care to patients and their families, we encourage you to apply for this position.

Our Admissions nurses work closely with our patient, their family, and other care teams to develop a comprehensive plan of care that meets the patient's individual needs. As this is many patients' introduction to hospice care, they will also provide education and support to the patient and their family.

For this role we will need a superstar to help our Director of Patient Care Services and Case Managers with our caseload.

Requirements:

• Hold Registered Nurse (RN) license
• Minimum of one (1) year of home health experience
• Possess and maintain a current CPR certification
• A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Duties:

• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.

$26 - $40 an hour
