Key Responsibilities\n

• Conduct in-depth psychosocial assessments for seniors to unravel and understand their emotional, mental, and social needs.

• Develop personalized care plans in collaboration with clients and their families, ensuring that each plan aligns with the clients' specific goals and desires.

• Provide counseling and emotional support, guiding seniors and their families through life transitions such as health declines, loss, and adaptation to hospice care.

• Act as an advocate for clients, ensuring their rights and access to necessary community resources and medical services are upheld.

• Engage with interdisciplinary teams, including nurses and healthcare professionals, to formulate and coordinate holistic care strategies.

• Regularly visit clients to monitor their emotional and physical well-being, reassess care needs, and address safety or environmental concerns.

• Facilitate family meetings to promote clear communication, resolve conflicts, and reinforce a unified approach to client care.

• Educate clients and families about relevant community resources, support networks, and available programs designed to enhance social engagement.

• Maintain comprehensive and accurate documentation of client assessments, care plans, and progress notes using electronic health record (EHR) systems.

• Stay current with best practices in social work, ethical standards, and regulatory compliance to uphold the quality of care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n

The position offers a competitive salary ranging from $90,000 to $100,000 annually, commensurate with qualifications and experience.
