Summary The Social Worker provides critical support to individuals and families navigating illness and disabilities. This role helps resolve personal, financial, and environmental challenges while following ADAPT Community Network policies, regulatory requirements, and professional standards. The Social Worker empowers families to understand and negotiate educational and clinical systems, advocating for their needs, and collaborates with other disciplines to ensure a comprehensive continuum of care and support. Essential Duties And Responsibilities Participates in the Adapt Intake Processes Develops case notes, case summaries/social histories, and appropriate recommendations for each student on caseload. Assists families in obtaining services such as housing, residential placement, food stamps, Medicaid, Assistive Technology services, respite services, family reimbursement services, camp, vacation referrals, etc. Integrates therapeutic intervention techniques into school programs or homes of students served through consultation with parents/guardians and staff at meetings, case conferences, or other informal contacts. Participates in interdisciplinary team meetings and annual reviews for planning, implementation, and monitoring of IEPs. Provides emotional and behavioral support to students, especially those with disabilities. Leads the school’s efforts to ensure students receive necessary social-emotional resources. Facilitates communication with family/caregivers concerning attendance, transportation, medical status, clearance, etc. Conducts parent trainings and monthly workshops in conjunction with multidisciplinary team members. Responds promptly to family concerns and provides guidance on available resources. Documents status of all active cases. Assists with duties such as arrival and dismissal, feeding, bathrooming, and classroom management as needed. Keeps abreast of changes and research in the field of Social Work. Ensures confidentiality of all information regarding students and families receiving services. Communicates with staff about goals, rights of recipients, confidentiality, family interaction, etc. Participates in activities related to student transitioning/Turning-Five Process. Serves as a member of interdisciplinary team meetings, annual reviews, parent meetings, Special Review Committee meetings, In-Services, etc. as required. Participates in in-service training programs as required. Provides supervision and training for Social Work interns and novice Social Workers. Attends professional meetings and conferences. Performs other related duties as requested. Qualifications Ability to read, write, speak, and understand English (Bilingual Spanish a plus). Effective oral and written communication skills. Good interpersonal skills to interact with employees, colleagues, students, families, and external contacts. Strong organizational skills and the ability to meet deadlines. Proficiency in personal computer use and relevant software. Maintains strict confidentiality due to the sensitive nature of the position. Ability to work independently and as part of a team. Ability to multi-task. Flexibility to work flexible hours. Sound judgment and ability to respond to unusual situations. Analytical skills to determine corrective actions. Must be available Monday‑Friday, 8 a.m.–3 p.m. Education and Experience Master’s degree in social work from an accredited program. NYS Licensed Social Worker with at least 1 year of prior experience preferred. License/Certification New York State Bilingual Spanish Extension. New York State Licensed Master Social Worker (LMSW). NOTE: Individuals with New York State Certification in Social Work (CSW) have certification automatically converted to the New York State Licensed Master Social Worker (LMSW).
