Advanced Practice Provider - Primary Care Shepard Park

Job Title Advanced Practice Provider - Primary Care Shepard Park (260001B1) Description The Advanced Practice Provider (APP) is credentialed by the medical staff and functions within their scope of practice in their jurisdictional area and in accordance with organizational medical staff bylaws. They are responsible for providing evidence-based care for the management of patients, including assessment, implementation and evaluation of a detailed plan of care, which incorporates diagnostic and therapeutic interventions. The APP demonstrates competency in providing age-appropriate care to patients as defined by certification and training and provides family centered care to meet family needs, including psychological support, education and discharge planning in conjunction with other related disciplines. The APP initiates, participates and collaborates with divisional and organizational multidisciplinary staff to provide and support evidence-based education, quality improvement and research. They participate in professional and organizational activities that contribute to improving patient care, the scientific knowledge base of advanced practice, program operation, and individual professional development. The APP provides leadership within their respective clinical area through participation in division projects and mentorship of others. They contribute to department and organization wide initiatives, representing advanced practice and promoting the role of the advanced practice provider. Qualifications Minimum Education Master's Degree Nurse Practitioner Master's or Doctoral degree as a Nurse Practitioner from an accredited program and eligibility for medical staff credentialing required. Master’s or Doctoral degree of Physician Assistant Studies from an accredited program and eligibility for medical staff credentialing required. Master’s or Doctorate degree in the clinical specialty area in which they practice from an accredited program and eligibility for medical staff credentialing required. Minimum Work Experience Minimum of 0 years of experience as an Advanced Practice Provider (Required) Required Skills/Knowledge Strong knowledge of clinical specialty and services. Demonstrates clinical leadership skills. Excellent written and verbal communication skills. Required Licenses and Certifications Advanced Practice Registered Nurse (APRN) Nurse Practitioner Licensure in the state(s) of clinical practice CDS in state(s) of clinical practice DEA Advanced Practice Registered Nurse (APRN) Clinical Nurse Specialist Licensure in the state(s) of clinical practice If prescribing controlled substances: CDS in state(s) of clinical practice DEA Nurse Practitioner Active national certification with governing body such as PNCB, ANCC, or AANPCB BLS certification Physician Assistant Active national certification with NCCPA BLS certification Functional Accountabilities Clinical Assesses, diagnoses, and monitors clinical status of each patient utilizing evidence based practice and medical knowledge. Develops appropriate plan of care. Writes patient care orders, prescribes therapy, and provides necessary home care orders as needed. Identifies significant changes in patient status using physical exam, lab values, diagnostic tests, and patient/family input. Differentiates between normal and abnormal findings. Performs procedures as needed after obtaining appropriate credentialing. Participates in and/or leads multidisciplinary patient care. Is a clinical resource for patient, parents and the interdisciplinary team. Appropriately communicates and escalates clinical/administrative issues. Collaborates with nursing, medicine, and other members of the healthcare team to regularly ensure quality patient care. Administrative Serves as a clinical preceptor to NP, CNS or PA students. Acts as a clinical resource for staff in patient care issues. Provides formal/informal educational presentation to staff on areas of expertise. Participates and leads quality improvement initiatives in their department. Organizational Accountabilities (Staff) Anticipate and respond to customer needs; follows up until needs are met. Escalates to management/leadership if needed to ensure customer concerns are heard and addressed. Teamwork/Communication Demonstrates collaborative and respectful behavior Partners with all team members to achieve goals Receptive to others’ ideas and opinions Performance Improvement/Problem-solving Contributes to a positive work environment Demonstrates flexibility and willingness to change Identifies opportunities to improve clinical and administrative processes Uses resources efficiently Manages financial resources and ensures accuracy of billing practices. Safety Appropriately escalates when team members appear to exhibit unsafe behavior or performance. Continuously validates and verifies information needed for decision making or documentation. Stops in the face of uncertainty and takes time to resolve the situation using expert resources. Demonstrates accurate, clear and timely verbal and written communication. Actively promotes safety for patients, families, visitors and co-workers. Attends carefully to important details - practicing Stop, Think, Act and Review in order to self-check behavior and performance.
