Nurse Practitioner (NP)
EGA Associates is seeking a qualified Advanced Practice Registered Nurse (APRN)/Certified Nurse Practitioner (CNP) to provide comprehensive primary care services at a Community Based Outpatient Clinic serving Veterans in and around McCurtain, OK. The APRN will provide patient-centered care to assigned and unassigned patients presenting for services within an outpatient primary care environment.
Working as a Primary Care Provider within the Patient Aligned Care Team, the APRN will assess and manage acute and chronic health conditions, develop clinically appropriate care plans, provide or arrange for needed services, and coordinate care with interdisciplinary team members. The APRN will practice in accordance with applicable licensure, certification, prescriptive authority, clinical privileges, scope of practice, and VA requirements.
Essential Functions
- Provide comprehensive primary care services to assigned and unassigned patients presenting for care.
- Evaluate, diagnose, treat, and manage acute and chronic medical conditions within the APRN's approved scope of practice and clinical privileges.
- Conduct patient histories, physical examinations, preventive health assessments, and clinically appropriate follow-up care.
- Develop and maintain individualized care plans containing appropriate medical recommendations for clinically indicated care.
- Offer clinically indicated healthcare services to patients assigned to the Patient Aligned Care Team.
- Provide or arrange for care to which the patient consents.
- Prescribe medications, including controlled substances when authorized, in accordance with federal law, state licensure requirements, clinical privileges, and applicable prescribing limitations.
- Order, interpret, and follow up on appropriate laboratory tests, diagnostic studies, medications, consultations, and referrals.
- Collaborate with physicians, nurses, mental health professionals, pharmacists, and other interdisciplinary team members to coordinate patient care.
- Develop personal health plans with patients and PACT staff that incorporate appropriate care management and care coordination.
- Provide clinical leadership to the care team and participate in the shared delegation of appropriate care activities to qualified team members.
- Review available clinical, access, quality, and performance data with the care team.
- Participate in continuous improvement activities focused on critical clinical and team-based care processes.
- Support same-day access to face-to-face and virtual care during regular clinic operating hours.
- Educate patients and caregivers regarding diagnoses, treatment plans, medications, preventive care, and health-management strategies.
- Maintain accurate, timely, and complete clinical documentation in accordance with VA, contract, and organizational requirements.
- Participate in quality assurance, chart review, peer review, clinical audits, and performance-improvement activities as required.
- Maintain all required licenses, registrations, certifications, prescriptive authority, credentials, and clinical privileges.
- Perform additional duties as assigned.
Qualifications
- Master's degree in nursing from a program accredited by the National League for Nursing Accrediting Commission or the Commission on Collegiate Nursing Education.
- Full and current national certification as a Certified Nurse Practitioner from the American Nurses Association or another nationally recognized certifying body.
- National certification in Adult Health or Family Practice.
- Current, full, active, and unrestricted license and registration as a graduate professional nurse in South Dakota, unless otherwise permitted under applicable VA qualification standards.
- Current authorization for prescriptive authority.
- Ability to prescribe controlled substances in accordance with the Controlled Substances Act, South Dakota licensure requirements, and applicable limitations on prescribing or administering controlled substances.
- At least three years of clinical nursing experience.
- At least one year of clinical experience practicing as a Certified Nurse Practitioner.
- Ability to successfully complete all required VA and EGA credentialing, privileging, background screening, and onboarding requirements.
- Clinical knowledge and experience necessary to provide comprehensive adult primary care.
- Knowledge of preventive care, chronic disease management, acute care, medication management, care coordination, and patient education.
- Ability to provide care consistent with approved clinical privileges and scope of practice.
- Ability to work effectively within an interdisciplinary, team-based primary care model.
- Strong verbal, written, and interpersonal communication skills.
- Ability to use electronic health records, telehealth platforms, and standard computer applications.
- Ability to maintain accurate, complete, and timely clinical documentation.
- Preferred: Three or more years of clinical experience practicing as a Certified Nurse Practitioner.
- Experience providing outpatient care in a Family Medicine or Internal Medicine environment.
- Previous experience providing healthcare services to Veterans.
- Experience working in a VA medical center, VA outpatient clinic, or contracted VA Community Based Outpatient Clinic.
- Familiarity with the VA Patient Aligned Care Team model.
- Experience with virtual care, telehealth services, quality improvement, chart review, or clinical performance monitoring.
Supervisory Responsibility
The Primary Care Provider does not have formal supervisory responsibility. The provider provides clinical leadership within the Patient Aligned Care Team, collaborates with interdisciplinary team members, and appropriately delegates care activities consistent with each team member's role, qualifications, and scope of practice.
Travel
Travel is expected to be negligible unless required for orientation, training, meetings, or other contract-related activities.

Perform all work in accordance with EGA policies, professional standards, contract requirements, and the spirit of EGA's mission. Adhere to EGA's Code of Conduct. Maintain all required licenses, certifications, credentials, privileges, and prescribing authorities. Protect patient privacy and comply with applicable confidentiality and information-security requirements. Complete required training, documentation, quality reviews, and administrative responsibilities within established timeframes. Immediately report patient safety events, compliance concerns, clinical risks, and other matters requiring escalation.
Expected Hours of Work
Your work schedule may vary depending on the contract with the Client. Your start time should be discussed and approved by your facility supervisor if you wish to deviate from the normal operating hours. Employees may be required to work additional hours in order to successfully complete work obligations. Employees are required to obtain prior approval from their facility supervisor before working overtime.
