Registered Nurse - Case Management- Case Manager

Job Details

RN-Case Manager jobs at the facility in Rapid City, SD let you coordinate patient care and support transitions across acute care settings. You will develop, implement, and evaluate individualized care plans, working closely with patients, families, and healthcare providers to ensure appropriate and affordable care.You must have an active South Dakota or compact RN license, a degree from an accredited nursing program, and at least 1.5 years of general nursing experience. Prior discharge planning and acute care case management experience are required.
