Travel Acute Care Case Manager / Discharge Planner
Schedule: - Weekend requirement: every other weekend - Holiday requirement: must commit to working 3 of these 5: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day
Assignment Details: - Caseload: up to 25 patients
Requirements: - Minimum 2 years of case management or discharge planning experience in an acute care hospital - California state license required; pending license accepted during process but must be active on start date - Experience with utilization review and care coordination - Familiarity with HMOs/IPAs, benefits eligibility, prior authorizations, CMS/DRG, Joint Commission/core measures - Charting system: Epic required; computerized documentation required - References: 2 references covering 1 year within the last 3 years required prior to start
Roles & Responsibilities: - Conduct patient assessments and develop discharge plans - Coordinate care and facilitate transitions to appropriate care settings - Arrange durable medical equipment (DME), skilled nursing facility (SNF) placement, and ambulance transport independently - Facilitate family meetings and contribute to treatment plans
Compliance / Notes: - Orientation: 40 hours - RTO restrictions: no more than 7 days - All travelers are expected to float between facilities within a state; refusal may result in termination or forfeiture of guaranteed hours
