You will manage a variety of health-related concerns, focusing on continuity of care, health maintenance, and disease prevention. This full-time position will be based at Conviva's San Jose center. As a Primary Care Nurse Practitioner on our team, you will: Provide comprehensive care to seniors in accordance with clinical standards Collaborate daily with your care team to ensure coordinated, high-quality care Manage referrals, hospital/SNF coordination, DME, and home health services Participate in daily huddles. CAs tend to lead the huddles Document care accurately and efficiently with support from quality-based coders Participate in on-call rotation and support growth initiatives at your center Ensure compliance with all licensing and accreditation requirements Use your skills to make an impact Requirements include: Master's Degree in Nursing with board certification Active, unrestricted NP license in the state of practice Minimum one year of adult medicine experience as an independently licensed NP Strong communication and interpersonal skills Passion for value-based care and integrated team-based models Basic computer proficiency (EMR, email, etc.) Preferred:Experience in value-based care, primary care or Medicare Advantage settings Preferred:Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers Preferred:Familiarity with HEDIS quality indicators and Medicare guidelines Preferred:Bilingual (English and Spanish) is a plus Compensation & Benefits Time with your patients Competitive base salary + annual bonus incentive Minimal Call Comprehensive benefits package - health insurance is effective on your first day of employment CME Allowance/Time Occurrence Based Malpractice Insurance Generous Relocation Assistance available Sign-on bonus opportunity 18 Days Paid Time Off (PTO) + 9 Paid Holidays + 1 VTO Day + up to 5 Personal Days (practitioners are offered 4 flex days per year) 401(k) with Employer Match Life Insurance/Disability Collaborative Team Environment and Career Cultivation #nursepractitionercareers#physicianassistantcareers#LI-KP1 Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year This job is eligible for a bonus incentive plan.
