Position Overview

Mawahib Recruiting is seeking an experienced and compassionate Case Manager for an opportunity in an ABA Clinic in Oakland County, Michigan . The Case Manager will be responsible for coordinating services, supporting clients in accessing appropriate resources, maintaining required documentation, and collaborating with providers and community partners to help ensure continuity of care.

The ideal candidate is organized, dependable, client-focused, and experienced working within the

Oakland Community Health Network (OCHN) system.

Key Responsibilities

• Coordinate and monitor services based on individual client needs and established plans.
• Connect clients with appropriate behavioral health, community, social, and support resources.
• Maintain accurate, timely, and compliant case documentation.
• Communicate and collaborate with clients, families, providers, and other members of the care team.
• Monitor client progress and follow up on services, referrals, and identified needs.
• Assist clients in navigating available programs and community resources.
• Participate in case reviews, meetings, and care coordination activities as required.
• Maintain confidentiality and follow applicable organizational, regulatory, and documentation requirements.
• Complete other case-management responsibilities as assigned.

Qualifications

• Minimum 3 years of relevant case management experience.
• OCHN experience is required.
• Experience working with behavioral health, community mental health, social services, or a related population preferred.
• Strong documentation, organization, and time-management skills.
• Excellent written and verbal communication skills.
• Ability to coordinate effectively with multiple providers and community resources.
• Ability to manage a caseload and prioritize client needs.
• Relevant degree, credentials, or additional qualifications may be required based on the employers program and position requirements.
