Physical Therapist - Full-Time

Seeking licensed Physical Therapists to provide personalized, in-home and community-based care to older adults. Responsibilities Deliver one-on-one therapy sessions in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments. Perform evaluations, develop individualized care plans, and document effectively using EMR systems. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists, while educating patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to enhance flexibility, access, and continuity of care. Enjoy autonomy in creating treatment schedules, building strong patient relationships, and practicing in diverse settings. Qualifications Valid Physical Therapy license, degree from an accredited program, computer proficiency, and CPR certification. Physical Demands Exert moderate effort, assist patient transfers, perform mobility activities, including driving to patient locations.
