Summary:

The role of the social worker is to deliver emotional and psychosocial support, along with education, to patients, families, and vulnerable populations. They provide therapy, counseling, care coordination, discharge planning, and advocacy. Working collaboratively within a multidisciplinary team, they practice at the highest level of their licensure, apply evidence-based approaches, and demonstrate flexibility across individual, group, and crisis interventions.


Responsibilities:

1. Completes patient assessment, reassessment and treatment plans as clinically indicated.

2. Ability to assess for risk and safety factors and be able to recommend next steps in treatment.

3. Provides clinical interventions which may include but not limited to counseling, individual sessions, groups, bereavement and crisis intervention as well as assessment and intervention in cases involving all family/patient dynamics.

4. Completes the required documentation and paperwork as it relates to compliance and other regulatory guidelines.

5. Works collaboratively with colleagues, fostering positive relationships within and beyond the team. Approaches challenges constructively and professionally, and partners with psychiatric staff and other health professionals to ensure high-quality diagnosis and treatment.

6. Establishes objectives and develops patient-focused treatment plans to provide disposition and/or referral. Identifies potential barriers, limitations & lack of coping abilities, adjustments, functional status.

7. Maintains knowledge of available community resources, evaluates for abuse concerns across populations, and ensures timely referrals.

8. Maintains, utilizes and applies evidence-based therapeutic practices, and treatment modalities.

9. Contacts and communicates with insurers, including managed care companies to clinically justify treatment to negotiate approval for clinical services.

10. Attendance of required department, patient and family meetings.

11. Maintains and Models Organizations Health Values.

12. Demonstrates regular, reliable and predictable attendance.

13. Performs other duties as required.


Education Skills Experience:

Requires at least 3 years of experience as an LCSW

Demonstrated clinical background and hospital or crisis experience, a combination of relevant internship and work experience can be considered.

Prefer: Certification or specialized in an evidence-based intervention (CBT, DBT, EMDR)

Skill Set:

ï½ Highly proficient in conducting diagnostic evaluations, developing comprehensive treatment and safety plans, performing suicide and risk assessments, managing crisis situations, and coordinating effective discharge planning

ï½ Proficiency in clinical reasoning and diagnosis formulation

ï½ Provides psychoeducation, crisis intervention, and individual, family, couples, and group therapy services

ï½ Highly organized with proven time-management skills

ï½ Demonstrates clear understanding of regulatory documentation standards and maintains compliance

ï½ Maintains current knowledge of, utilizes and applies evidence-based therapeutic practices, and treatment modalities

ï½ Ability to work independently with minimal supervision
