BASIC FUNCTION The Pharmacist in Charge (PIC) - Class D Clinic Pharmacy is responsible for the lawful, safe, and compliant operation of the assigned Texas Class D clinic pharmacy or pharmacies. The PIC provides continuous pharmacist supervision of pharmacy-related activities; develops and maintains the clinic formulary and pharmacy policies and procedures; oversees procurement, storage, provision, dispensing, records, security, and quality controls; conducts or ensures required on-site pharmacist visits and staff training; and serves as a member of the clinic Pharmacy and Therapeutics Committee. The role ensures compliance with the Texas Pharmacy Act and Texas State Board of Pharmacy (TSBP) rules governing Clinic Pharmacy (Class D), including 22 TAC §§291.91-291.94 and other applicable rules. PRIMARY RESPONSIBILITIES AND DUTIES
- Serve as Pharmacist in Charge for the Class D clinic pharmacy
- Provide continuous supervision of pharmacy-related activities and personnel
- Lead formulary governance and the Pharmacy and Therapeutics Committee
- Oversee procurement, storage, security, equipment, and drug accountability
- Control prepackaging, labeling, provision, dispensing, delivery, and compounding
- Develop, implement, and maintain the Class D policy and procedure manual
- Perform required pharmacist visits, inspections, and regulatory oversight
- Maintain Class D pharmacy records and document retention
- Oversee temporary clinic sites when applicable
- Provide pharmacy leadership, quality, and interdisciplinary collaboration

2) Participates in quality-improvement, medication-safety, and regulatory-readiness activities.
3) Provides education and coaching to clinic personnel within the scope of the PIC role.
- Advanced knowledge of Texas Pharmacy Act and TSBP Class D clinic pharmacy requirements.
2) Knowledge of formulary management, drug procurement, storage, labeling, provision, dispensing, drug information, recordkeeping, and pharmacy inspection standards.
3) Ability to apply professional judgment, identify compliance risk, develop corrective actions, and communicate requirements to interdisciplinary clinic teams.
4) Ability to organize and oversee multiple Class D clinic sites when assigned.
- Current, unrestricted Texas pharmacist license in good standing.
2) Must be eligible for designation as Pharmacist in Charge of a Texas Class D clinic pharmacy.
3) Must execute and maintain the written PIC agreement required by 22 TAC §291.92(a)(1)(B).
- Proficient with pharmacy information systems, electronic health records, inventory and wholesaler systems, electronic regulatory records, and standard office applications.
2) Able to maintain audit-ready regulatory documentation, formulary records, training records, visit logs, drug-accountability records, and temperature/storage records.
- Communicates regulatory and medication-safety requirements clearly to pharmacists, nurses, providers, technicians, clinic leadership, and other supportive personnel.
2) Able to document findings, recommendations, corrective actions, and P&T Committee decisions accurately and professionally.
