Role Description This full-time, on-site RN Home Health Clinical Manager role is based in Midlothian, VA and is eligible for a sign-on bonus. The Clinical Manager oversees day-to-day clinical operations, including supervising nursing staff, coordinating patient visits, and ensuring high standards of home health care. This role reviews and approves care plans, monitors patient progress, and collaborates closely with physicians, therapists, and other care team members. Responsibilities include ensuring regulatory compliance, supporting staff with clinical decision-making, and promoting best practices in documentation, safety, and infection control. The Clinical Manager also participates in performance improvement initiatives, mentors team members, and helps optimize patient satisfaction and clinical outcomes. Qualifications Strong clinical expertise in Nursing and Medicine, with the ability to provide guidance, oversight, and support to a multidisciplinary home health team. Demonstrated experience in Home Care, including care coordination, home visit workflows, and understanding of home health regulations and standards. Proficient in Patient Care, including assessment, care planning, patient and family education, and monitoring of clinical status in the home setting. Hands‑on skills in Wound Care, including evaluation, treatment planning, and ongoing management of acute and chronic wounds in collaboration with providers. Active, unrestricted RN license in the state of Virginia (or eligibility to obtain), with prior home health or acute care leadership experience preferred. Strong organizational, communication, and documentation skills, with proficiency in electronic health records and visit scheduling systems. Ability to lead, coach, and motivate a diverse clinical team, while fostering a culture of respect, accountability, and continuous improvement.
