CareNational is seeking a skilled and compassionate Physical Therapist (PT) for a full-time contract with a well-regarded Long-Term Care/Post-Acute Rehab facility in Grants Pass , OR . This is an excellent opportunity for a licensed therapist who enjoys helping residents regain mobility and independence while working a consistent weekday schedule.

Pay Details:
$52.86 - $55.44 per hour

Onsite | Rehab Department - Long-Term Care / Post-Acute Facility

Position Details:
Location: Grants Pass, OR
Schedule: Monday through Friday
Shift: Variable Shift
Hours: 40 Hours per Week
Type: Full-Time Contract
Start Date: ASAP

Certification Requirements:

• Physical Therapist License - Oregon (Required)

• BLS Certification (Required)

Skill Requirements:

• Licensed to practice as a Physical Therapist in Oregon

• Ability to develop and implement individualized treatment plans

• Strong clinical assessment and documentation skills

• Effective problem-solving and decision-making abilities

• Excellent communication and interpersonal skills

• Prior experience in long-term care, rehab, or post-acute settings preferred

Key Responsibilities:

• Evaluate residents to determine therapeutic needs and potential

• Create, implement, and modify treatment plans as necessary

• Provide direct therapy services in accordance with physician-approved plans

• Document evaluations, treatments, progress notes, and charges accurately

• Participate in discharge planning to support ongoing patient success

• Supervise Physical Therapist Assistants and Aides

• Screen residents for potential benefit from physical therapy interventions

• Ensure compliance with facility, state, and federal standards
