Position Overview

We are seeking a dynamic, high-energy Registered Nurse (RN) to join our team as a Home Health Clinical Manager. If you thrive in a fun, fast-paced environment and love guiding clinical teams toward excellence, this is the role for you. You will have responsibilities in reviewing documentation, mentoring field staff, and ensuring top-tier patient care.

What You Will Do

Lead Teams: Supervise, mentor, and evaluate a dedicated team of home health clinicians.
Manage Care: Coordinate patient admissions, care plans, and daily clinical assignments.
Ensure Compliance: Monitor clinical documentation to meet federal, state, and local regulations.
Drive Quality: Conduct case conferences and quality assurance reviews to optimize patient outcomes.
Solve Problems: Adapt quickly to changing patient needs and operational demands in a fast-paced setting.

Required Qualifications

Licensure: Active, unencumbered Registered Nurse (RN) license in the state of practice.
Home Health Experience: Minimum of 1 year of direct home health nursing experience.
Leadership Experience: Minimum of 1 year of clinical management or supervisory experience.
Education: Associate degree in Nursing required; BSN preferred.
Basic Requirements: Valid driver’s license, reliable transportation, and active auto insurance.

Preferred Qualifications

Software: Experience using Homecare Homebase (HCHB) electronic medical record software.
