Pay Range: $90,000/annually, DOE. Job Type: Full-Time. Schedule: Friday evening – Monday morning. Horizon Home Health & Hospice seeks nurses in the Meridian, Idaho area who are compassionate, have strong clinical skills, and enjoy being part of an amazing team. Essential Job Functions / Responsibilities Patient Care: Complete initial, comprehensive, and ongoing hospice assessments of patient and family to determine needs. Provide complete physical assessment and history. Complete ‘as needed’ assessments for home health and palliative patients. Provide professional nursing care using all elements of the nursing process, assess and evaluate patient status, participate in care plan revision, use assessment data to determine nursing diagnosis, contribute to care plan goals, include patient and family in planning, initiate preventive and rehabilitative procedures, administer medications and treatments as prescribed, counsel patient and family, adhere to documentation and care procedures, provide health instructions, assist with activities of daily living and facilitate independence or comfort care. Act as Case Manager until assigned. Communication: Complete, maintain and submit accurate clinical notes; record pain/symptom management changes; communicate with physicians regarding patient needs and changes; obtain orders; teach self‑care techniques; provide medication, diet and other instructions; collaborate with interdisciplinary group; maintain safe environment; assist team in providing continuity of care. Additional Duties: Perform all on‑call duties per policy; ensure equipment and service arrangements; supervise ancillary personnel and delegate; pursue professional growth via continuing education and in‑service classes. Qualifications Registered Nurse with current licensure in the state. Graduate of National League for Nursing accredited nursing school. Maintains current CPR certification. Licensed driver with insured automobile in good working order. Minimum two years’ experience, at least one year in public health, home care, or hospice nursing preferred. Self‑directed with minimal supervision; no management experience required. Must supervise home health aide for weekend visits if assigned. Excellent observation, problem‑solving, verbal and written communication; nursing skills per competency checklist; ability to organize and prioritize workload independently. Physical Requirements & Work Environment Prolonged walking or standing. Ability to lift, position, and transfer patients and supplies. Reach, stoop, bend, kneel, or crouch.
