Registered Nurse - Home Hospice

Schedule

This is a PM position, working from 11 AM to 7 PM, and includes every other weekend.

Job Description

The Registered Nurse will perform patient assessments, develop care plans, and ensure appropriate utilization of hospice services. The ideal candidate will possess prior experience in a hospital setting and demonstrate a solid understanding of hospice regulations and insurance guidelines.

Responsibilities

• Conduct thorough patient assessments and determine eligibility for hospice care.
• Develop and communicate individualized care plans for patients and families.
• Ensure compliance with visit productivity standards and manage appropriate service utilization.
• Perform discharge assessments and maintain accurate electronic medical records.
• Utilize age-appropriate teaching methods to educate patients and families on care plans.
• Assume responsibility for case management, revising care plans as necessary.
• Participate in interdisciplinary team meetings every two weeks.
• Ensure a safe environment by following infection control measures and medication management protocols.
• Engage in performance improvement activities and provide mentorship to new employees.

Required Experience / Certifications / Licensure

• Minimum of two years of RN experience in a home hospice environment.
• Current New Jersey RN License and a valid New Jersey driver's license.
• BLS certification required.
• Experience with electronic medical records (EMR) systems.
• Strong critical thinking and communication skills.

Assignment Duration

This is a 13-week contract position.
