Accepted Documents (1 of each): Mortgage statement or Lease agreement Utility Bill: Electric, Gas, Water, Sewer, or Internet (linked to the physical residence) TN/compact licensure and any required certs/licensed must be in hand at time of submittal Facility will not allow certifications/licensure to expire within 45 days of the start date. Please note if candidates are pushed the same rule applies and the certs/licensure may also not expire within that 45-day period. Candidates should start working on renewals if within that time period.
