Responsibilities
- Conduct psychosocial assessments of patients and families to identify needs, barriers, and discharge planning requirements.
- Develop and coordinate discharge plans, ensuring timely placement and communication with the care team and outside resources.
- Provide counseling and support to patients and families facing medical illness, treatment decisions, and transitions in care.
- Document assessments, care plans, and progress notes accurately in the electronic medical record; adhere to regulatory and privacy standards.
- Collaborate with an interdisciplinary team during rounds and care planning to optimize patient outcomes and safety.
- Identify social determinants impacting care and connect patients with appropriate community resources and supports.

Requirements
- Active Georgia LCSW license
- AHA BLS certification
- Minimum 2 years of recent experience in medical social work or related specialty
- Acute inpatient care setting experience
- Ability to work in a fast-paced inpatient environment
- Strong documentation, communication, and collaboration skills
