Physical Therapist

Role Licensed Physical Therapist responsible for providing personalized, one-on-one care to older adults in private homes and senior living communities, supporting mobility, independence, and quality of life. Key Responsibilities Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy Support patient wellness, functional independence, and longevity through therapy sessions Document patient progress accurately using electronic medical records Collaborate with interdisciplinary teams including OT, SLP, and Fitness Specialists Educate patients, caregivers, and staff to promote safety and aging in place Utilize telehealth and virtual care to extend access and flexibility Qualifications & Skills Valid Physical Therapy license or eligibility in the practice state Degree from an accredited Physical Therapy program Proficiency with computers and EMR systems Current CPR certification Physical Requirements Ability to exert moderate effort, assist with patient transfers, perform CPR, and drive between locations, with physical demands including lifting, repositioning patients, and extended walking or standing. #J-18808-Ljbffr Apex Home Health, Hospice & Home Care
