The Public Health Nurse provides professional nursing services to individuals, families, and population groups to promote health, prevent disease, reduce risk, and improve health outcomes across the community. This position applies public health nursing principles through assessment, case management, care coordination, health education, and community collaboration, with a focus on vulnerable and high-risk populations. The Public Health Nurse works collaboratively within multidisciplinary teams and community partnerships to support population-based interventions, communicable disease surveillance and response, health equity initiatives, and emergency preparedness activities. Services are delivered using trauma-informed, evidence-based practices and in accordance with applicable laws, professional standards, and Public Health policies.

Essential Duties

• Provide public health nursing services to individuals, families, and population groups using prevention-focused, trauma-informed, and evidence-based practices to promote health, reduce risk, and improve health outcomes across the community.
• Conduct comprehensive nursing assessments of individuals and families, including health history, physical and developmental status, psychosocial functioning, environmental factors, and access to basic needs and health care; develop, implement, and evaluate care plans in collaboration with clients and partners.
• Perform case management, care coordination, referral, follow-up, advocacy, and health education for vulnerable and high-risk populations, adjusting plans as needed to support treatment adherence, disease prevention, and recovery.
• Conduct communicable disease surveillance, investigation, and follow-up in accordance with Wisconsin Statutes and Public Health reporting requirements, including timely initiation of investigations, contact tracing, isolation and quarantine guidance, and documentation in required state surveillance systems.
• Administer nursing interventions as ordered or authorized, including vaccinations, specimen collection, blood draws, injectable medications, and other clinical services consistent with licensure, protocols, and training.
• Participate in community health assessment, health improvement planning, and population-level interventions; analyze health data to identify trends, disparities, and emerging risks and contribute to program planning, performance management, and quality improvement initiatives.
• Develop and deliver culturally responsive, health-literate education and outreach to individuals, families, organizations, and community groups using a variety of methods, including in-person presentations, printed materials, digital platforms, and media.
• Collaborate with multidisciplinary and multi-agency partners—including healthcare providers, behavioral health staff, schools, correctional systems, community-based organizations, and coalitions—to coordinate services, strengthen community resilience, and advance health equity.
• Maintain accurate, timely, and confidential documentation in electronic health and surveillance systems in compliance with HIPAA, regulatory requirements, professional standards, and agency policies.
• Participate in internal and external meetings, workgroups, coalitions, and action teams related to assigned program areas; contribute to development, review, and revision of policies, procedures, and standard operating protocols.
• Support emergency preparedness and response activities in alignment with CDC Public Health Emergency Preparedness Capabilities, including participation in response teams, clinics, and disaster response activities as assigned and trained.
• Complete required training, continuing education, professional development, and competency maintenance, including licensure requirements, clinical certifications, and public health program training.
• Perform other related duties as assigned consistent with job classification.

Qualifications

Minimum Education: Bachelor's or Master's degree from an approved/accredited nursing program. Minimum Experience: Two (2) years of public health nursing experience preferred. A combination of education and experience may be considered. Minimum Certifications: Licensed as a Registered Nurse under WI State Statute Chapter 441. National Incident Management System (NIMS)/Incident Command System (ICS) 100, 200 certification within four (4) months of employment. CPR certification within one (1) year of employment. Other Certifications may be required after employment. A Valid Wisconsin Driver's license. Other Requirements: Bilingual language skills are preferred.

• Knowledge of public health nursing principles, population health concepts, disease prevention, health promotion, and community-based interventions applicable to diverse populations.
• Knowledge of or experience with trauma-informed principles and practices, including recognition of trauma impacts and application of respectful, supportive approaches when working with individuals, families, and communities.
• Skill in collaborating effectively within multidisciplinary Public Health teams, including Nutritionists, Sanitarians, Public Health Educators, Public Health Nurses, Community Health Workers, and other internal and external partners to coordinate services and achieve shared health outcomes.
• Skill in providing culturally responsive, compassionate, and respectful nursing services that support individuals' and families' needs, goals, preferences, cultural traditions, family situations, and values.
• Effective interpersonal and communication skills to build honest, reliable, and transparent relationships with coworkers, consumers, members of the public, and representatives of other agencies; ability to communicate clearly in both oral and written formats.
• Demonstrated professionalism, integrity, courtesy, and respect in all interactions, with the ability to foster a work environment that is inviting, approachable, and helpful to others.
• Skill in maintaining accurate, timely, and confidential documentation in accordance with HIPAA, professional standards, and Public Health policies.
• Proficiency in navigating computer software systems, including Microsoft Office and Google Workspace applications, electronic health records, and state-required documentation and reporting systems.
• Sound judgment, adaptability, and emotional composure when working with individuals experiencing health concerns, stress, or crisis situations.
• Ability to work independently and collaboratively, manage competing priorities, and contribute positively to team-based and community-focused Public Health initiatives.

Work Environment & Physical Requirements

• Work is performed in a general office environment within Public Health facilities and in community-based settings, including client homes, workplaces, schools, and other public locations.
• Position requires the ability to be fitted for and wear appropriate personal protective equipment, including respiratory protection, as required for assigned duties.
• Work involves a range of physical activities, including climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, lifting, grasping, feeling, writing, and performing repetitive motions.
• Position requires exerting up to 30 pounds of force occasionally and/or a negligible amount of force continuously to move objects such as supplies, equipment, or records.
• Hearing sufficient to communicate effectively with individuals in person, by telephone, and in group or clinical settings is required.
• Vision sufficient to read printed and electronic materials, view computer screens, perform clinical assessments, and safely operate a motor vehicle is required.
• Community-based work may involve exposure to varying environmental conditions, including inclement weather, temperature extremes, snow and ice, noise, wetness or humidity, uneven surfaces, and limited lighting conditions.
• Community settings may present exposure risks including blood-borne pathogens, tuberculosis, other communicable diseases, insects and other disease vectors, hazardous materials, chemicals, environmental toxins, and animals.
• Employees may occasionally interact with individuals who exhibit challenging, atypical, or hostile behaviors and must maintain professionalism, situational awareness, and personal safety.
• Risk of exposure to blood-borne pathogens is considered high.
